If you are a first-time patient for either Chiropractic or Nutrition, please allow yourself between an hour to complete the necessary paperwork and questionnaires. If you have health insurance, please make sure that you bring all necessary insurance cards at the time of your appointment.

Patients who have insurance can let us know about their coverge before their initial visit so we can verify the benefit information. Unless otherwise arranged, any deductible or co-payment is required at the time services are rendered.
